Social Prescribing /Care Coordination aims to increase people’s active involvement in their local communities. It can strengthen resilience at both a personal and community level, and it reduces health inequalities by supporting people to address the wider determinants of health, such as debt, poor housing and physical inactivity. It works particularly well for people with long-term conditions (including support for mental health), for people who are lonely or isolated, and those with complex social needs which affect their wellbeing You will be responsible for linking patients to services across the Health, Statutory and Voluntary sectors in the area. You will work alongside patients to help them achieve their holistic goals, undertaking assessments and building relationships to agree personalised plans, which will empower the patient to make the necessary changes to enable them to live the life they choose. You will work closely with GPs, community services, Social Care and Practice staff and wider MDTs. Ideally you will have worked in a similar role, but equally this role will be suited to someone with good local knowledge, appropriate experience and enthusiasm.
