COVID Vaccination Evidence
The Practice has seen a large increase in the number of patients requesting access to medical records on the NHS app as evidence that they have had the Covid vaccinations.
If you require access to evidence you have been vaccinated, you should bring photographic evidence to reception who will process the request within three working days.
If you do not have access to a smart phone and require evidence then, after 17 May, you should contact 119 who will send you a letter confirming you have had both vaccinations within 5 working days.
