Third Party Confidentiality Agreement

Between you and The Village Practice

Introduction:

This agreement aims to ensure that any contractor or third-party required to work on organisation premises is aware of the confidentiality requirements of the organisation.

Agreement:

The above, when visiting The Village Practice, will make themselves aware of their duties and responsibilities with regard to confidentiality as set out below. The organisation reserves the right to refuse access to the above if, in the organisation’s opinion, there is any doubt about that person’s commitment to abide by this confidentiality agreement.

Visiting the organisation:

All visitors must report to reception on arrival. Visitors must remain in the presence of a member of organisation staff at all times while they are on the organisation’s premises unless otherwise agreed by a manager. Other than carrying out your duties whilst on the organisation’s premises, you must refrain from engaging in unnecessary conversation or gossip either with organisation staff or with members of the public who may or may not be known to you. The very fact that a person is visiting the organisation is confidential and should not therefore be discussed with anyone at any time.

Any information, no matter how irrelevant or trivial it may appear, which is seen or overhead while visiting or working at the organisation must not be discussed with or disclosed to anyone else (including organisation staff and/or attached staff and/or other contractors) under any circumstances other than in carrying out your duties.

Agreement:

The above, hereby agrees to conform to the policy set out above when The Village Practice and acknowledges that any breach of confidentiality may be grounds for termination of this and any subsequent visits to the organisation.

Fire safety and risk awareness

All visitors and contractors are to please read the fire safety and current risk information provided. This identifies the actions required to be undertaken.

The Village Practice is committed to providing a safe environment for its employees, contractors, visitors and members of the public.

As a visitor to the organisation, you will be unfamiliar with the fire safety procedure and the layout of the building. Additionally, you should be aware of the current risks that may impact upon your visit. It is therefore requested that you spend time reading this safety information.

Fire detection, fire warning and firefighting equipment

The organisation has fire safety systems installed and fire protection measures throughout the premises to protect all persons, building and contents. If you notice that there is any damage to the fire safety equipment, please advise a member of staff.

Should you require a personalized evacuation plan please make a manager aware.

The fire panel is located in reception and the alarm is tested every Wednesday. Staff will advise prior to sounding. Upon hearing any alarm, unless previously informed of a test, you are to safely evacuate the building, along with any patients and muster at the evacuation point which is at the muster point in the main patient car park. If you believe that you may have difficulties in evacuating the building, you are to discuss this with the reception team.
